Yes. Allpoints Home Health Care is a Medicare-certified agency (CMS provider #157541) and has served Northwest Indiana families since 2005. Medicare certification means we meet federal standards for quality and patient safety and can bill Medicare directly for covered services.

For patients who qualify, Medicare covers eligible home health services, often with little or no out-of-pocket cost. Actual coverage depends on your plan and medical eligibility. Before care begins, our team reviews your benefits with you so there are no surprises. We also work with many other insurance plans, call us to confirm yours.

Care usually begins with a referral from your physician, but you can start the conversation with us any time. Call (219) 922-9595 or request a callback, and we will coordinate with your doctor to set up your plan of care.

Once we have your physician’s order and have confirmed your coverage, we work to begin care promptly. Timing depends on your individual situation, call us and we will give you a realistic timeline.

Visit frequency is set by your physician’s plan of care and your individual needs. It may range from several visits a week to less often as you progress. Your care team explains your schedule and adjusts it as your needs change.

Yes. You have the right to choose your home health provider. If you would like Allpoints, simply let your discharge planner or physician know, we will handle the coordination from there.

Home health is skilled, physician-ordered clinical care focused on recovery and managing a condition. Non-medical home care (help with bathing, meals, and errands) and hospice (comfort-focused end-of-life care) are different services.
